Centralized Care Coordination
JPHSA’s Centralized Care Coordination (CCC) works with external agencies to connect individuals to our programs. CCC ensures referred individuals can access and stay engaged with the services and supports they need.
What We Do
- Simplify access to services by using a “no wrong door” approach
- Link individuals to resources, supports, and services
- Coordinate clinical appointments for referred individuals
- Complete follow-up to promote continued engagement
- Help providers, treatment teams, and service recipients communicate and stay in touch
- Provide one point of contact for questions and concerns
- Promote quality of care, timeliness of service delivery, ease of access, and overall satisfaction
