JPHSA’s Health Centers, JeffCare- East Jefferson and JeffCare- West Jefferson, offer primary care, mental health, addictive disorder, and integrated treatment services.  Services are offered in a variety of forms including health promotion, disease prevention, health maintenance, recovery maintenance, patient education, diagnosis and treatment of acute and chronic illnesses including mental illness, and counseling on an individual or group basis.

The philosophy of the Health Centers is one of wellness and recovery which drives our holistic approach to the provision of services.

 Primary care services include:

  • Routine health screenings;
  • Treatment for colds, flu, and other illnesses;
  • Management of hypertension, diabetes, cholesterol, obesity, and other chronic conditions;
  • Annual wellness checks;
  • Immunizations;
  • Health education;
  • Referral and care coordination; and,
  • Behavioral health medication maintenance.

 Behavioral Health services include:

  • Bio-psychosocial assessments;
  • Psychiatric evaluations;
  • Medication management;
  • Individual therapy;
  • Group therapy;
  • Early intervention (birth through age 5) for high-risk children;
  • Intensive outpatient treatment for an addictive disorder;
  • Crisis intervention;
  • Referral to higher levels of care as appropriate; and,
  • Health and Wellness Education.
